Provider Billing Information

Log into the Vendor Information Portal (VIP), Choices’ Online Billing System, by clicking here.

Upon joining Choices’ Provider Network you will receive a user ID for the VIP. If you need assistance accessing the Vendor Information Portal please contact Provider Technical Support at ProviderTechnicalSupport@choicesteam.org or 317-472-8424.

Medicaid Care Coordination

Illinois Choices is a state-funded program to serve the needs of youth with physical, behavior, social, and mental health challenges.

Only youth (ages newborn to 21) who are on Medicaid (also known as All Kids) and meet the following conditions are eligible for Choices. They must:

  • be or have been part of the Screening, Assessment and Support Services (SASS) Program;
  • be receiving or have received behavioral health services and/or mood changing medication;
  • have threatened to harm himself or herself, or others in the past two years; and
  • live in Ford, Iroquois, Champaign and Vermilion counties.

Choices works with families and HFS to assess eligibility. The program is voluntary; youth and families do not have to participate in the program and can leave at any time during the first 90 days.

Illinois Choices coordinates a variety of medical and social support services for youth signed up for the program. Some of the services that Choices will provide include:

  • working with members of the team to decide what services should be offered;
  • ensuring youth get the health examinations and immunizations they need;
  • authorizing payment for behavioral healthcare services through Medicaid;
  • identifying and coordinating additional services and supports through various community resources;
  • working with providers to make sure that they have up-to-date information about the youth’s care; and
  • teaching providers and families about the program and health-related issues

Medicaid Rights, Responsibilities, and Grievances

We want to provide you with the best possible service. When something goes wrong or you are not treated well, please let us know. We take grievances very seriously. For more details on your rights, responsibilities and how to file grievances, please refer to your handbook. If you have a grievance, you should first contact your care coordinator, who will make best efforts to informally address the issue. However, if your grievance cannot be resolved through this means, you have other options to do so.

Should you need to file a grievance, you may do so via our private, dedicated response line at hfsgrievance@choicesteam.org. You may also submit a grievance in person, by phone, or by mail, using the contact information to the right.

If you need to appoint a representative to act on your behalf during the grievance process, please contact us.
