Montgomery County IFPS

Eligible youth in Montgomery County are those facing challenges related to the following:

  • substance abuse;
  • truancy;
  • frequent running away;
  • behavioral challenges in the home, classroom, or community;
  • family conflict;
  • mental health challenges;
  • imminent risk for out-of-home placement or meet at least four of the risk factors identified on the referral form; and
  • youth who are transitioning to their homes from out-of-home placement or are being discharged from hospitalization.

Targeted Case Management and 1915i Care Coordination

Youth (ages 3-18) with Medicaid and a mental health diagnosis who need coordinated mental health services in Caroline, Dorchester, Kent, Queen Anne’s, and Talbot counties may be eligible for Targeted Case Management (TCM). Depending on the needs of the youth, they may receive services at one of three levels – Level 1 is the least intensive and Level 3 is the most intensive. Youth may also be eligible for 1915i care coordination and additional specialized services. Many times, youth in this program have experienced difficulties at home, school, or in the community.

Families served by TCM have a dedicated care coordinator who helps them work through family challenges and crises, connects families with other local resources, and navigates complex systems to get the youth additional services he or she needs in order to be successful.
