We partner with families, schools, providers, government agencies, and managed care organizations to customize care for individuals and their families and bring about equitable outcomes.

National studies show that youth receiving care coordination are more likely to pass classes and progress in school, more likely to graduate from high school, less likely to be arrested, and less likely to end up in juvenile detention.
