This month marks a powerful milestone for our Ohio team three years of delivering care coordination through OhioRISE (Resilience through Integrated Systems and Excellence). Since launching the program in July 2022, Choices Coordinated Care Solutions has supported over 1,900 youth and families across Montgomery and Preble counties through customized, community-based mental health care.
OhioRISE is designed to help youth with complex behavioral health needs receive the right care, in the right place, at the right time. Through this initiative, our dedicated team works hand-in-hand with families, schools, healthcare providers, and community partners to ensure wraparound care that prioritizes stability, healing, and long-term success.
“This program is about more than services it’s about showing up for families who need a team in their corner,” said a member of our OhioRISE team. “Reaching over 1,900 families in just three years speaks to the real need and the real impact this work brings.”
To celebrate this incredible achievement, our team hosted a special event filled with food, fun, and a 360° photo booth experience. Staff came together to reflect on the journey so far and renew our shared commitment to supporting youth across the region.
