Choices Coordinated Care Solutions is a national nonprofit organization committed to supporting individuals who are facing significant behavioral and emotional challenges.
Choices offers care coordination, therapy services, and evidence-based treatment models across our many programs. We customize care for each person by partnering with government agencies, healthcare providers, educators and professionals from managed care and community organizations.
Intercept
Intercept intensive in-home services focuses on proven ways to help families in their homes, schools, and communities. Intercept works with doctors, teachers, case workers and courts to manage their child’s behavior themselves.
Crisis Continuum
We offer Community Brief Crisis Support Services (CBCS) for individuals under 21 through Medicaid, providing up to 15 days of follow-up care to ensure crisis resolution and connection to ongoing supports.
Wraparound
Wraparound is a team-based planning process that places youth and their families at the center while building on strengths and find lasting resources within their communities.
