Wraparound for Louisiana youth and their families

Choices serves Louisiana youth who are experiencing significant emotional or behavioral challenges.

We do this through a process called Wraparound.

Wraparound helps youth and their families build on strengths and find lasting resources within their communities.

About youth we serve

Youth we work with have complex mental or other behavioral health needs. Many are involved with systems such as child welfare and juvenile probation. Our goal is to help youth thrive in healthy, loving, and stable homes.

Choices currently serves youth who:

Are 5 through 20 years old
Live in Louisiana Serving Region 3 (Hammond and surrounding area) and 6 (Lake Charles and surrounding area)
Have a mental health diagnosis or exhibit high risk behaviors
Are eligible for Louisiana Medicaid
Meet other assessment criteria

What is Wraparound?

Wraparound is a team-based planning process that places youth and their families at the center. Learn more below, or visit the National Wraparound Initiative website for more information.

Documenting Strengths

Choices works with a youth and their family to build a team that includes service providers and supportive relatives or friends. With the team’s support, each family identifies its strengths and takes the lead in developing its vision for the future.

Planning for the future

Together, the group develops and follows a plan of care to help the youth and family improve their lives. Once the formal Wraparound process is over, families have resources and a natural support system in place to flourish within their communities.

Our Partners

Louisiana Choices serves youth who are at risk of or living in out-of-home care and are enrolled in the Louisiana Department of Health’s Coordinated System of Care (CSoC).

Magellan Health Services and Healthy Louisiana determine eligibility of a referred youth for Wraparound.
