All youth and families Choices partners with are engaged with multiple systems including mental health, child welfare, and juvenile probation.

Traditional health care involves a doctor or counselor telling youth and families how to solve problems or treat a disease. When the youth and family is left out of the planning, it may not set them up to succeed.
With care coordination, youth and families participate in decision-making and planning with the support of a team. Care coordination uses strengths to help youth achieve goals and plan for their future.
Care coordination connects youth and families with local resources and services to meet their specific needs. They also have the freedom to choose service providers that best fit their culture and preferences.
Youth who receive care coordination are stronger, more stable, and more likely to remain with their families.

Choices has a contractual partnership with the Indiana Department of Child Services (DCS) to provide Cross-System Care Coordination services for youth with complex needs. The youth referred to Choices are involved with DCS and/or juvenile probation.
We receive referrals across the state from DCS. Check with your DCS Family Case Manager or Probation Officer to see if a Choices Cross-Systems Care Coordination referral is appropriate.
