Documents
Copies of the following documents/items are required in the licensing process:
- Driver’s license for applicants;
- Birth certificates for all household members;
- Income verification (pay stubs or tax forms, pension statements, Social Security income, etc.);
- Auto registration verification;
- Auto insurance verification;
- Water bill (or water test results, if on well water);
- Gas and/or electric bills; and
- Telephone bill(s).
